Why do teeth feel sensitive in the cold?

What it is

Cold drinks or cold air can cause a sudden, sharp pain in certain teeth, especially if they are sensitive or exposed.

How it works

Inside each tooth is a soft inner tissue called the pulp, which contains:

  • Nerves
  • Blood vessels
  • Connective tissue

Normally, this area is protected by:

  • Enamel (hard outer layer)
  • Dentin underneath
  • Gum tissue around the roots

1. Cold changes fluid movement inside the tooth

Dentin contains microscopic tubes called dentinal tubules.

When something cold touches the tooth:

  • Fluid inside these tiny tubes contracts and shifts rapidly
  • This movement stimulates nearby nerve endings

The brain interprets this sudden nerve signal as:

Sharp pain or sensitivity

This is called the hydrodynamic theory of tooth sensitivity.

2. Exposed dentin makes the effect stronger

Sensitivity increases if enamel or gums are worn down because:

  • The protective barrier becomes thinner
  • More tubules are exposed to temperature changes

Common causes:

  • Brushing too hard
  • Gum recession
  • Tooth grinding
  • Enamel erosion from acids

3. Cold creates a fast nerve response

Cold temperatures trigger nerves very quickly because the tooth is highly sensitive to sudden thermal change.

That’s why the pain is often:

  • Sharp
  • Immediate
  • Brief

Rather than dull and long-lasting.

Why it matters

This explains:

  • Pain from ice cream or cold drinks
  • Sensitivity when breathing cold air
  • Why exposed roots hurt more
